Services and SubsidiesThe Arizona Chapter of APDA will help members pay for four types of services: exercise classes, Response Link, Caregiver Holiday and massage. Click this link to download an application. For each program, you will pay a portion of the cost and AZ-APDA will pay a portion. Fees are based on a sliding scale. To be eligible, you must pay annual dues of $25 per year to the Arizona Chapter of APDA. Services are provided anywhere in the state of Arizona that a qualified provider can be identified. Services are provided in part by grants from the Del Webb Foundation, the Eliot Spalding Foundation and the William E. Schmidt Charitable Foundation. Click here to download an application. After filling out the application, fax or mail it to AZ-APDA. Exercise classesThere is growing evidence that exercise can help keep Parkinson’s symptoms at bay. Exercise also can help you feel better mentally and emotionally. If motivation is a problem, joining a class can help you stay on track. Most of the exercise classes listed charge a monthly fee to participants. Fees vary, but the average cost is $50 per month to attend class twice a week. AZ-APDA will pay half the cost of the class up to $25 per month. You pay your portion directly to the instructor and AZ-APDA will pay the balance. Check the calendar for classes. ResponseLinkResponseLink is a system which includes a personal help button worn around the neck or wrist and a unit that dials the ResponseLink Center whenever you press the help button. The Center is staffed by individuals trained to determine what kind of help you need and then arrange assistance. You pay the one-time installation fee of $25 and AZ-APDA splits the cost of the monthly monitoring fee (you pay $17.50 per month and the AZ-APDA pays $17.50 per month). Caregiver HolidayThe Arizona Chapter of APDA will subsidize the cost of 4 hours per week
of in-home care through a licensed agency so the family caregiver may have a
break from caregiving duties. The cost of in-home care is approximately $22
per hour. A brief financial questionnaire is used to determine what your
share of cost will be. Participants will pay between $6 and $10 per hour and
AZ-APDA will pay the balance. Care in an adult day healthcare program can
also be arranged for up to 8 hours per week at roughly half these rates. |
